In the 2003 version lost episode 20, Atom knows he can't grow up and wonder if he'll have friends even if he stay like a child robot forever. Turns out that he would still have friends regardless if he can't age.
Also, when I imagine a older Atom like 20 years or more, the freaky image of Namor: The Submariner come in mind.
Thank good we don't got the Astro Boy for nes and Snes during the bad box art era.
Imagine a adult looking Atom who seemed like Namor in these bad box art covers? O_O

I Googled that character up and eeew! I do NOT picture adult Astro as some kind of "beastly" looking creature.
Just look at how Tezuka drew him in Atom is Stolen - strong but NOT "muscle bound." (also no nipples or 6-pack-abs)
Based on the changes he made to Astro's appearance in the manga covering his rescue/repair by the aliens, I think he would modify Astro's appearance as an adult to be more "modest" and not exaggerated. Astro's "power" isn't flamboyant and obvious. And because he's a robot, he doesn't HAVE to have physical "muscles."
